MUST BE FILLED OUT 48 HOURS PRIOR TO YOUR APPOINTMENT
- ALL CLIENTS ARE LIMITED TO ONE RESCHEDULE OUTSIDE OF THE ONE WEEK TIME FRAME, IF GIVEN A 72HR NOTICE.
- ANYTHING LESS THAN 72HRS WILL RESULT IN FORFEIT OF YOUR DEPOSIT.(THIS INCLUDES SICKNESS OR ANY OTHER THINGS THAT MAY COME UP DAY OF APPOINTMENT.)
- IF YOU RESCHEDULE FOR A SECOND TIME, YOU WILL FORFEIT YOUR ORIGINAL DEPOSIT AND WILL HAVE TO RE-BOOK WITH A NEW DEPOSIT.
- IF YOU CHOOSE TO CANCEL YOUR APPOINTMENT ALL TOGETHER YOU WILL FORFEIT YOUR DEPOSIT AUTOMATICALLY.

I UNDERSTAND THE FOLLOWING:
I understand Brow Lamination is a process of restructuring the brow hairs to keep them in a desired shape but it is your responsibility to brush your brows daily to the desired look. Results may vary.
I realize that there is possibility of allergic reactions to the products used.
Brow Lamination is NOT recommended if you have the following:
1.Psoriasis/Eczema
2.Alopecia
3.Sunburn
4.Pregnant/Breast feeding
5.Super sensitive skin
6.Allergic to perm solution
7.Allergic to hair dye
I understand that Navarre Aesthetics LLC has the right to stop contact with the client and may refuse to provide a service for any reason.
I have been given the opportunity to ask questions about the procedure, the risks, and the hazards involved and have read over pre/post care.
I believe that I have been given sufficient information to give this informed consent. I understand that there will be no refund given and that results may vary.
I understand that it is my responsibility to advise the technician of any concerns I may have before they begin the procedure, even though I may have written it down on the form.
I understand that taking before and after pictures is a condition of such procedures. I hereby forever release and discharge Navarre Aesthetics from any and all claims, actions, and demands arising out of use of said photographs for website and social media.
BROW LAMINATION PRE-CARE
Discontinue retinol, AHA, BHA, vitamin-c serums, etc or any other prescription topicals for your skin. We strongly recommend consulting a professional (dermatologist) prior to your appointment.
For best results, please grow your eyebrow hairs and avoid trimming/threading/waxing/plucking for up to 4 weeks prior.
***I do not offer waxing but I do clean up the brow area with a razor. If you prefer waxing I suggest doing so 4 days prior to your appointment.***
It is important for this area to be clean and free of makeup, pressed powders and moisturizers
BROW LAMINATION AFTERCARE
Aftercare is extremely important following this service as poor aftercare, in extreme cases, can result in the loss of brow hair.
Do not wet your eyebrows for 24-48 hours (NO DIRECT WATER IN THE SHOWER, NO SWIMMING, NO saunas.)
NO SWEATING AKA WORKING OUT FOR THE FIRST 24 HOURS
Avoid touching the brow area at the risk of transferring oils
Do not apply makeup to the brows for 24 hours post-treatment
Do not apply Retin A, aha or exfoliate the brow area for 72 hours before or after the treatment
No self-tanner products should be used on the face for at least 48 hours post-treatment
Apply castor oil nightly starting 24 hrs after service (failing to do so can result in dry brow hair)
Comb the hairs into place each day
It is recommended to schedule brow lamination treatments 8-12 weeks apart.
Must wait 6 weeks in between booking lamination appointments.
